Adult/Child Waiver
If you’re using Union Point Sports and are unable to provide a Certificate of Insurance (COI), all participants are required to complete the waiver at the link below. This ensures proper coverage and is mandatory for facility use.
The waiver helps protect both participants and the facility by clearly outlining the risks associated with athletic activities. Without a COI or completed waivers, groups will not be allowed to take the field. We appreciate your cooperation in helping us maintain a safe and compliant environment for everyone.
